- Be a patient of Cherry Tree Dental Care Blackburn and awaiting your scheduled appointment with our Dentist. Walking past the surgery does not qualify.
- Keep the check-in on your timeline until we have completed the Marketing of the competition winner
- Allow us to create promotional material for marketing purposes that include a photograph of you collecting your prize
- The competition closes on 28th February 2017
- The competition winner will be chosen at random on March 15th 2017
- The competition winner will be notified by either telephone, email or Facebook Messenger.
- The competition winner must collect the prize from the practice at 376 Preston Old Road, Blackburn, Lanashire, BB2 5LL
